For new baby registration, a parent/guardian must complete form HS123 - provided by the registration office at the time of registration of the child's birth - and present it to our reception staff.

For persons registering with a doctor in the UK for the first time, please complete form HSC/R1. For persons transferring from another UK practice, please complete form HS200. Both of these forms are available at our reception.

We also ask that you fill out this new patient questionnaire at the time of registration.
